Session V Information
Boys Baseball, Girls Fastpitch, and Track & Field

Session V begins Monday, April 15th.

IMPORTANT INFORMATION - SEE THE DOCUMENTS TAB

Deadline to get ALL forms in for tryouts is Friday,
April12th


It is your responsibility to make sure you have an updated physical on file at the school and all eligibility forms are completed and turned in before the session begins. 
Forms can be picked up at the main office.

You must have the following completed before the first day of practice/tryouts:


Proof of Physical
(good for 13 months)

Must be on file with the school nurse (571-7087)

 

Middle School Activities Contract & Eligibility Form

This year they are YELLOW (Only need one for the year)
            Parent and athlete must COMPLETE and SIGN BOTH SIDES
            Turn in to the office

Concussion Information Form

Color is WHITE (Only need one for the year)
      Turn in to the office

Purchase ASB ($15)

Emergency Medical Information Card

Half Sheet of Paper
            Color is RED
           
This card is given to the coach for EACH session

All grade levels are eligible to try out for each sport.

**  Students must have a 2.0 GPA with no failing grades throughout the season to be eligible to participate in inter-school competition. *This eligibility will be determined by the school’s regular grading cycle or formal progress reports. Review of each student’s academics will be done at a minimum of every other week during each season, monitored by the athletic director. Students ineligible based upon the school’s regular grading cycle or formal progress reports will be placed on suspension from competition for one week. If at the end of one (1) week, a formal progress report for the student reflects that he/she is meeting the requirements, he/she may then be reinstated for competition. Only one progress report will be allowed per week. If the student does not meet the requirements, he/she will remain ineligible and on suspension from competition. During the suspension period, the student will be allowed to practice but will not be allowed to play in a game until the student meets the academic standard stated above.

 

*The academic standard may be waived by the building administration upon written request by the teacher and/or athletic director.
